A named product, route, population and condition.
Spravato has specific adult depression indications and a supervised REMS program. Approval does not spread from one product or use to everything called psychedelic medicine.
The altered state is not the evidence.
Ten drugs and drug families are often placed under one bright word. Their approval status, evidence and medical oversight are not remotely the same.
Evidence and regulatory status checked August 28, 2026; registry status checked August 31, 2026. Licensed-prescriber and legal review are pending; no review badge is claimed.
Spravato has specific adult depression indications and a supervised REMS program. Approval does not spread from one product or use to everything called psychedelic medicine.
Ketamine injection is approved as an anesthetic. Psychiatric use of the approved injection is off-label; FDA has not approved that use for depression, PTSD or anxiety.¹⁰ ¹²
A compounded ketamine product is not FDA-approved and has no FDA-approved indication or route. Ask which product, formulation, route and pharmacy are involved.¹³
Psilocybin, MDMA, LSD, DMT, methylone, ibogaine and the sponsor-coded compounds on this desk remain investigational. An IND, Phase 3 record or development designation is not approval.¹
Psilocybin and MDMA remain investigational and federally controlled. So do the LSD, DMT, ibogaine, methylone, ayahuasca and mescaline routes named below. FDA-approved ketamine injection may be prescribed off-label for psychiatric care; compounded ketamine products are not FDA-approved. Esketamine has specific FDA-approved depression uses and a required supervised safety program. “Decriminalized,” “breakthrough,” and “allowed to enter a trial” do not mean “approved treatment.”¹ ² ³ ¹⁰ ¹² ¹³
Psilocybin is not FDA-approved. A registered study is the research route. A paid “facilitation” service is not FDA-approved treatment, and this dated official-source review identified no statewide California psilocybin service-center system.
→02InvestigationalMDMAMDMA remains federally controlled and is not FDA-approved for PTSD. Established PTSD care and registered trials remain the treatment routes.
→03Approved injection · psychiatric use off-labelKetamineFDA-approved ketamine injection is an anesthetic. Psychiatric use of the approved injection is off-label; compounded ketamine products are not FDA-approved. The exact product, route, prescriber, monitoring, emergency plan and full course price are part of the decision.
→04FDA-approved usesEsketamineSpravato has FDA-approved adult depression indications. It is administered in a certified setting with at least two hours of monitoring—not dispensed for use at home.
→05InvestigationalLSD / lysergideCalifornia research includes investigational lysergide for depression and anxiety. A recruiting registry record does not create approved LSD-assisted therapy.
→06InvestigationalDMT / 5-MeO-DMTA California DMT study is designed for healthy participants. No FDA-approved DMT or 5-MeO-DMT mental-health treatment exists.
→07InvestigationalIbogaine / noribogaineA recruiting California ibogaine record observes people with MRI and EEG; it does not provide ibogaine treatment. An allowed IND is not approval.
→08InvestigationalMethyloneA California phase 3 PTSD study is recruiting. Methylone remains investigational and the study decides eligibility.
→09No statewide CA system identifiedAyahuascaThere is no FDA-approved mental-health use. This dated official-source review identified no statewide California licensed-treatment system or recruiting California treatment study.
→010No statewide CA system identifiedMescalineThere is no FDA-approved mental-health use. This dated official-source review identified no statewide California licensed-treatment system or recruiting California treatment study.
→Randomized phase 2 studies reported reductions in depression scores after supervised psilocybin with psychological support. The trials were small compared with ordinary drug-approval programs, selected participants carefully, and do not establish a safe consumer service for everyone. One treatment-resistant-depression study did not show a sustained response through 12 weeks on its key sustained measure.⁴ ⁵
FDA’s July 2026 final guidance exists because psychedelic trials have unusual problems: masking is difficult, psychological support may affect the result, effects can last for hours, and safety monitoring matters. Guidance for research is not approval of a drug.¹
Compare the evidence by exact substance and condition →California’s statewide decriminalization bill, SB 58, was vetoed in 2023. A local decision to make enforcement a lower priority does not create FDA approval, a statewide service-center system, or a California health-care licence.⁷ ¹¹
A licensed therapist may help you discuss risks or make sense of a difficult experience without supplying or administering a controlled drug. Ask where psychotherapy ends, who prescribes or supplies the substance, which licence governs each person, and who is medically responsible during the drug session. “Psychedelic facilitator” alone answers none of those questions.
Test the words in an offer →Phase 3 trials reported improvement in PTSD symptoms with MDMA-assisted therapy. MDMA nevertheless remains unapproved and federally controlled. FDA convened an advisory committee on the application in 2024; an advisory meeting and a development program do not themselves authorize clinical use.³ ⁶
If you need PTSD care now, compare established trauma treatments. If you want research participation, verify the study in the federal registry and read its eligibility, location, sponsor and recruitment status yourself.
LSD / lysergide: California phase 3 depression research includes recruiting and not-yet-recruiting sites. Other anxiety and depression studies remain active but are not recruiting. The drug is investigational.
DMT and 5-MeO-DMT: a La Jolla DMT record is not yet recruiting healthy participants for a neural-effects study. The active California 5-MeO-DMT record is observational, combines several interventions, and is not recruiting.
Ibogaine and noribogaine: FDA allowed an early-phase noribogaine IND to proceed in 2026; the agency explicitly says that permission to study is not approval or a finding of safety or effectiveness. The recruiting UC Irvine ibogaine record is observational and does not supply the drug.⁸
Methylone: this entactogen is not MDMA. A phase 3 PTSD study lists recruiting California sites, but methylone remains investigational.
Ayahuasca and mescaline: no recruiting California treatment study appeared in the August 28 registry snapshot. Neither has an FDA-approved mental-health use, and this dated official-source review identified no statewide California licensed-treatment system for either one.
Open the California clinical-trial desk →Use ClinicalTrials.gov. Confirm the official study contact on the registry, then ask about eligibility, randomization, placebo or comparator, travel, costs, compensation, follow-up and who pays for an injury. Never pay a third party merely to put you on a “trial waitlist.”
We do not rank providers, repeat testimonials, or call a local enforcement policy a treatment licence. Future directory results will use your filters and verified facts. Payment will never purchase a higher position.
This dated official-source review identified no statewide licensed psilocybin or MDMA therapy system in California. FDA-approved ketamine injection may be prescribed off-label for psychiatric care; compounded ketamine products are not FDA-approved. Spravato has specific FDA-approved adult depression uses and a supervised REMS program. A city’s enforcement policy does not turn psilocybin into approved medicine or a licensed treatment service. This is a general regulatory distinction, not a legal judgment about a particular business.¹⁰ ¹² ¹³
